摘要
目的 研究可溶性髓样细胞表达的触发因子受体-1(sTREM-1)在类风湿关节炎(RA)患者血清中的表达及其与临床指标的相关性,探讨TREM-1及其sTREM-1在RA发病中的意义.方法 选自2010年10月~2012年10月就诊于邯郸市第一医院风湿免疫科的RA患者80例为RA组,同时选取健康体检者74例为健康对照组.比较两组的关节肿胀数、DAS28评分、血沉(ESR)、类风湿因子(RF)、X线分期差异.结果 RA组血清sTREM-1水平为(3.19±1.28)ng/mL,健康对照组为(2.55±1.41)ng/mL,两组比较差异有统计学意义(t=2.94,P<0.05);两组按性别划分的血清sTREM-1水平差异无统计学意义(P>0.05);血清sTREM-1和RA患者的关节肿胀数、X线分期、ESR、RF和抗CCP抗体呈正相关,而与病程、年龄、晨僵时间、关节压痛数、超敏CRP和关节功能状态不相关;在RA患者中,稳定期组和活动期组的血清sTREM-1水平差异有统计学意义(P<0.05).结论 sTREM-1与RA患者关节肿胀数、DAS28评分、ES、类RF、X线分期正相关.sTREM-1与RA患者发病年龄、晨僵时间、病程长短、压痛关节数、超敏CRP和关节功能状态无明显关系.病程的不同阶段,血清中sTREM-1水平亦不同.
Objective To study the expression of serum levels of sTREM-1 in patients with rheumatoid arthritis and its correlation with clinical indicators; to explore its significance, to provide a new train of thought for further in-depth study of the pathogenesis of RA and its clinical treatment. Methods 80 patients with rheumatoid arthritis treated in the First Hospital of Handan City from October 2010 to October 2012 were selected as RA group. At the same time, 74 cases healthy physical examination were selected as normal control group. The number of swollen joints, DAS28 score, blood sedimentation (ESR), rheumatoid factor (RF), X-ray stage in two groups were compared. Results Serum sTREM-1 level of RA patients were (3.19±1.28) ng/mL, the normal control group were (2.55±1.41) ng/mL, the differences were statistically significant (t = 2.94, P 〈 0.05); the difference of serum sTREM-1 level between the sex group was not statistically significant (P 〉 0.05); positive correlation were found between serum sTREM-1 of RA patients and swollen joints, X-ray stage, ESR, RF, anti-CCP antibody, no correlation was found between serum sTREM-1 of RA patients and course of the disease, age, time of morning stiffness, joint tenderness, hypersensitivity; the differences of serum sTREM-1 level between plateau group and the activity group was statistically significant (P 〈 0.05). Conclusion sTREM-1 has positive correlation with the number of swollen joints, DAS28 RA patients score, ESR, RF, X-ray stage, has no correlation with RA onset age, morning stiffness time, course length, number of tenderness joints, allergic CRP. As stage of the course is different, the sTREM-1 in serum levels are also different.
