周疗法去铁治疗慢性铁超负荷的临床观察

Clinical observation of iron removal treatment with week-therapy of chronic iron overload

目的探讨周疗法去铁治疗输血相关性慢性铁超负荷的临床应用及疗效。方法以20例慢性铁超负荷患者为研究对象，回顾性分析周疗法静脉滴注铁螯合剂去铁胺治疗输血相关性铁超负荷患者的去铁效果及治疗前后肝功能、胰腺功能情况。结果治疗6个月后，患者血清铁蛋白（SF）水平为（2693．7±709．9）μg／L，较治疗前的（2771．5±725．2）μg／L无明显变化，但临床症状明显减轻；治疗12个月后，SF水平为（2083．7±714．4）μg／L，与治疗前水平比较差异有统计学意义（t=3．35，P〈0．01），其中16例患者下降明显；7例患者治疗后肝功能改善；5例胰腺功能好转，血糖下降。结论应用去铁胺周疗法治疗慢性铁超负荷患者能降低血清SF水平，且可使受损脏器的功能得到改善。

Objective To explore the clinical application and curative effect of iron removal treatment with week-therapy in chronic iron overload related to blood transfusion. Methods Twenty patients who were diagnosed as chronic iron overload were retrospectively studied. The amelioration of liver and pancreatic fanctions in patients with chronic iron overload treated with week-therapy of iron chelating agent through intravenous drip. Results The changes in the level of serum ferritin [2693.7_＋709.9） p,g/L] were not significant after treatment at six months, but the cilinical symptoms were relieved obviously. The levels of serum ferritin in patients after the treatment at twelve months were significantly different from the level before treated [（2083.7＋714.4） p,g/L vs （2771.5__.725.2） [xg/L, t = 3.35, P 〈 0.01], and the decreases were significant in sixteen patients. The liver functions in seven patients were improved and pancreatic functions were better in five patients. Conclusion The study suggests that iron removal treatment with week-therapy is effective for chronic iron overload, and improves the functions of damaged organs.
