高危人乳头瘤病毒、p16、p53蛋白在胃癌和癌前病变中的表达

Detection of high risk human papillomavirus, p16 and p53 proteins in gastric carcinoma and precancerous lesions

目的 探讨高危HPV感染与抑癌基因表达产物p5 3、p16蛋白在胃癌发生、发展中的作用及与肿瘤预后的关系。方法 应用原位杂交和免疫组化技术 ,分析 96例胃癌和癌前病变及腺癌组织中HPV 16 /18DNA、p5 3和p16蛋白的阳性表达情况。结果 HPV 16亚型和 18亚型的阳性率分别为 34 38%和 8 33% ;p5 3和p16蛋白阳性表达率分别为 46 88%和 43 75 %。HPV 16亚型、p5 3和p16蛋白在癌前病变中的阳性表达率与腺癌组织有显著差异 (P<0 0 5 ) ,HPV 16和p16蛋白、p5 3蛋白之间具有显著相关性 (P <0 0 5 )。结论 HPV 16亚型感染、p5 3及p16蛋白的水平表达与胃癌的呈正相关 。

Objective To investigate the roles of HPV-16/18 infection and p16 and p53 expression in the pathogenesis and development of gastric carcinoma and their prognostic value. Methods In situ hybridization and immunohistochemical staining were used to detect HPV-16/18 and p16 and p53 proteins in 96 cases of gastric carcinoma as well as precancerous lesions. Results Positive rates of HPV-16 and HPV-18 were 34% and 8% differently; Expression of p16 and p53 was seen in 47% and 44% cases of the tumor respectively. There was a significant difference ( P <0.05) in the expression of p16 and p53 between gastric carcinoma and the precancerous lesions. HPV-16 infection was closely related to aberrant expression of p16 and p53 ( P <0.05). Conclusions Infection of HPV-16 subtype and the expression of p16 and p53 in different levels are positively correlated with the development of gastric carcinoma. Decreased expression of p16 can be regarded as a poor prognostic marker for the patients.
