经动脉栓塞后切除下肢巨大血管瘤

Resection of giant hemangiomas in lower extremity after transarterial embolization.

目的 探讨治疗巨大血管瘤的有效方法。方法 对 1995～ 1999年收治的 111例下肢血管瘤或血管畸形中的 3例巨大臀部高流量血管瘤的治疗经过进行回顾性分析。结果 巨大的臀部高流量血管瘤经数字减影血管造影 ,继而进行有效的动脉栓塞后 ,病变体积缩小 ,3例常规手术难于治疗的血管瘤得以成功地完整切除 ,避免了术中致命性的出血 ,并缩短了手术时间。结论 术前经动脉栓塞是根治性切除巨大血管瘤的重要步骤。

Objective To investigate the effective therapy of giant hemangiomas.Methods Of 111 patients admitted and treated for hemangiomas or vascular malformations in lower extremity between 1995 and 1999,three cases with giant,high-flow gluteal hemangioma were analyzed retrospectively.Results By digital subtraction angiography and effective arterial embolization,the mass of lesion shrank,avoiding fatal uncontrollable bleeding during surgery and enabling a shortening of operating time.All of three giant gluteal hemangiomas were successfully resected,of which,the routine surgical removal could be very difficult.Conclusion Preoperative transarterial embolization plays an important role in total excision of giant hemangioma.
