微血栓监测在急性缺血性脑卒中的应用价值

摘要 目的在急性缺血性脑卒中的临床应用价值。方法选取228例急性期缺血性脑卒中患者,发病5d以内,应用德国DWL公司双通道双深度TCD监测双侧大脑中动脉,30min/次,声强5dB,选取至少跨越血管狭窄段适当的深度,取样容积≤双深度距离。回放时人工识别栓子信号并计数。结果 13例检出微栓子信号,阳性率5.7%,明显低于以往报道。结论微血栓监测在检测急性缺血性脑卒中是有意义的,但作为常规检测阳性率较低。 Objective To explore the evaluation of microembolus (MES) in diagnosis and prediction of acute ischemic stroke by analysis 228 patientg MES. Methods The bilateral middle cerebral artery of 228 patients with acute ischemic stroke within 5 days after the onset of stroke were monitored by using multigated transcranial Doppler system (TCD) examinations(30 minutes each time, 5 dB sound intensity, proper depth crossing vascular stenosis). Embolus signal was be manually identified and counted and all TCD examinations were analyzed by the same observer. Results Thirteen cases were detected to be MES and the positive rate was about 5.7 %, which was lower than that before reported. Conclusion MES monitor is a method mo- nitoring acute ischemic stroke, however, the positive rate is still lower for the conventional detection.
