锁定加压钢板结合髂骨植骨治疗肱骨干下1/3骨不连

Locking compression plates and autologous iliac bone struct in the management of ununited of the distal third humerus shaft fracture

目的探讨锁定加压钢板结合髂骨植骨治疗肱骨干下1/3骨不连的疗效。方法对19例肱骨干下1/3骨不连患者,取出原内固定,自体髂骨植骨,锁定加压钢板(LCP)固定,术后行X线片检查和肩、肘关节功能评估。结果本组19例均随访8个月~33个月,平均15.6个月,所有患者术后均骨折愈合,平均愈合时间4.9个月(3个月~8个月),术后并发桡神经麻痹2例。根据X线片及肩肘并节功能疗效判定:优13例,良4例,差2例,功能优良率为89.5%。结论肱骨干骨不连的最主要原因是固定装置不稳定,采用锁定加压钢板(LCP)结合髂骨植骨可提高固定强度,促进骨折愈合,能够获得较好的肩、肘功能。

Objective To investigate the clinical outcome of the locking compression plates and autologous iliac bone struct in the management of ununited of the distal third humerus shaft fracture. Methods 19 cases of the ununited distal third humerus shaft fracture were managed removal of ori^nal internal fixation ,internal fixation with locking compression plates （LCP）and autologous iliac bone struct. The outcomes were assessed by radiographs and evaluation of shoulder and elbow function. Resulls The mean duration of follow-up was 15.6 months （8~33months）,all cases acquired bony union with a mean healing time of 4.9 months（3~8months）, temporally palsy of the radial nerve occurred in 2 cases.The total excellent and fine rate of shoulder and e]blow joint function was 89.5%.Conclusions Fixing device instability is the main reason for ununitad of the humerus shaft fracture, the treatment of locking compression plates and autologous iliac bone smlct can improve the fLxed intensity enormously, improve bone healing and ensure good functional recovery of the upper extremity.
