探讨B超影像尿动力学检查在下尿路功能障碍患儿诊断中的应用价值

To explore the application value of B ultrasound urodynamic examination in the diagnosis of children with urinary tract dysfunction

目的探讨B超影像尿动力学检查在下尿路功能障碍患儿诊断中的应用价值。方法选取来我院进行治疗的下尿路功能障碍患儿54例,作为观察组,选取同期由于肾积水需要进行B超影响尿动力学检查的患儿15例,作为对照组。两组患儿均进行B超影像尿动力学检查。结果观察组患儿的B超影像尿动力学检查表现出了残余尿量增多,膀胱顺应性降低,逼尿肌括约肌协同失调,逼尿肌瘫痪;膀胱壁厚度增加、毛糙,膀胱颈口发生抬高,尿道扩张,在排尿期其尿道不能完全开放等现象。对照组患儿的B超影像尿动力学检查可见膀胱壁光滑,膀胱壁的平均厚度为(2±1)mm,膀胱颈口关闭,不抬高,排尿期膀胱颈口和后尿道充分开放。结论 B超影像尿动力学检查能够显著的提高小儿下尿路功能障碍临床诊断的准确率和全面性。

Objective To explore the application value of B ultrasound urodynamic examination in the diagnosis of children with urinary tract dysfunction. Methods To our hospital for children with lower urinary tract dysfunction in 54 cases, as the observation group, select the same period due to hydronephrosis needs of children with B ultrasound urodynamic examination in 15 cases, as control group. Two groups of B ultrasound urodynamic examination. ResuRs The group of children with B ultrasound urodynamic examination showed residual urine volume increased, decreased bladder compliance, detrusor sphincter dyssynergia of detrusor muscle paralysis,; bladder wall thickness increased, coarse, bladder neck inflated, urethral dilatation, completely open in voiding the urethral not phenomenon. Patients in the control group by B ultrasound urodynamic examination visible bladder wall is smooth, the average thickness of the bladder wall （mm, 2 ± 1） for bladder neck down, not up, micturition period bladder neck and urethra fully open. Conclusion Ultrasound imaging of urine dynamics test can significantly improve the accuracy in pediatric clinical diagnosis of urinary tract dysfunction under and comprehensiveness.
