摘要
目的:臭氧联合胶原酶融核术治疗腰椎间盘突出症的临床研究。方法:34例经临床诊断为腰椎间盘突出症患者,行个体化介入治疗方案,在透视及CT引导下经皮穿刺至责任椎间盘内和/或硬膜囊外侧隐窝、椎间盘突出物表面下、椎管内游离髓核,单独或联合运用臭氧消融术与胶原酶化学溶解术,术后对症处理及功能锻炼,并经系统完整随访。结果:观察患者术后6个月的治疗效果,根据改良的Macnab方法评价疗效,其疗效优、良、差率分别为82.0%、15.0%、3.0%。结论:影像引导下个体化注入臭氧和胶原蛋白酶联合应用是一种微创介入治疗腰椎间盘突出症的微创、有效、安全的方法,值得临床推广。
[ Objegtive] To research the clinical effect of image - guided percutaneous injection of ozone and collagermse to treat lumbar disc herniation. [Method] 34 patients with clinical diagnosis of lumbar disc herniation were treated with individualized intervention program. With fluoroscopy and CT patients were percutaneous punctured to intervertebral disc and/or &wal sac lat- eral recess, mtervertebral disc protrusion surface, nucleus pulposus in spinal canal was dissociated. Surgeries were carried out with ozone ablation and coUagenase chemical dissolution alone or combination. The postoperative symptomatie treatment, func- tional exercise and fl.up were carried out after surgery. [Result] After 6 months of treatment, the effect were observed with Macnab method Tb_e rate of better, good and poor efficacy is 82.0%, 15.0% and 3.0%, respectively. [ Conclusion] Individ- ualized image- guided injection of ozone and collagenase joint application is a minimally invasive interventional treatment of lumbar disc herniation. It is a invasive, effective, safe and worthy of promotion.
