老年患者中低位硬膜外阻滞初量的回顾性分析

A retrospective analysis of initiative dose of lower epidural block in aged patients

目的：探讨老年患者中低位硬膜外阻滞初量的使用规律。方法：对本院120例中低位连续硬膜外阻滞的临床资料和初量作回顾性分析。其中：41例20~50岁中青年患者为对照组（Ⅰ组）;79例老年患者按年龄分成2组,60~69岁组（Ⅱ组,N=45）,70岁及以上组（Ⅲ组,N=34）。所有患者均在T11~L2之间椎间隙行硬膜外穿刺、置管。结果：初量用量Ⅰ组（14.7±1.1）ml,Ⅱ组（12.4±1.9）ml,Ⅲ组（11.2±1.9）ml。3组初量均不相等,差异有统计学意义（P〈0.05）。二次给药和分次给药数、低血压发生率,3组差异无统计学意义（P〉0.05）,不同性别的老年患者初量差异无统计学意义（P〉0.05）。结论：老年患者中低位硬膜外阻滞初量随年龄增长而逐步减少,60~69岁为对照组的84%,70岁及以上为对照组的76%。

[ Objective] To investigate the initiative dose of lower epidural block in aged patients. [ Method] One hundred and twenty patients underwent lower epidural block were divided into three groups according to the age： group Ⅰ （2- 50 years old, n = 41）, group Ⅱ （60- 69 years old, n = 45） and group Ⅲ （more than 70 years old, n = 34）. All cases underwent epidural tube at T11 - L2 space. Initiative dose and clinical data were analyzed retrospectively. [ Result ] Initiative dose was （14.71.1）ml in Group Ⅰ （ 12.41.9）ml in group Ⅱ and （ 11.21.9）ml in group Ⅲ. There were significant differences be- tween the three groups（ P 〈 0.05）. There were no significant differences in the incidence of hypotension and twice or more ad- ministration among these three groups（ P 〉 0.05）. There were no significant differences between the male and the female in aged（ P 〉 0.05）. [ Condusion] It needs less initiative dose of lower epidural block in the aged.
