原发性胆汁性肝硬化患者对UDCA治疗的生化应答标准 被引量:2

摘要 原发性胆汁性肝硬化(PBC)是一种慢性肝内胆汁淤积性疾病,其发病机制与自身免疫有关。该病多见于中年女性,临床症状主要表现为乏力、瘙痒,实验室检查以碱性磷酸酶(ALP)、谷氨酰转肽酶(GGT)升高为主,血清学可见特异性抗线粒体抗体2型(AMA—M2)阳性,病理特征为慢性非化脓性胆管炎,主要累及小叶间隔及间隔胆管。熊去氧胆酸(UDCA)是目前唯一被美国食品及药品监督管理局批准的用于PBC治疗的药物。多项研究报道UDCA治疗可以改善PBC患者的生化指标,但能否改善组织学、远期生存率及降低肝移植需求尚无统一结论”。近几年研究结果显示,UDCA治疗生化应答良好的患者生存率明显高于应答欠佳者。
