摘要
目的研究脑胶质瘤侵袭性的超微结构基础。方法将45例手术病理证实的星形胶质细胞瘤患者分为恶性胶质瘤(病理分级Ⅳ级、Ⅲ级)和低度恶性胶质瘤(病理分级Ⅱ级、Ⅰ级)两组,并收集颅脑外伤需减压手术所得的正常脑组织标本5例,作为对照观察对象。分别制片并应用透射电镜观察三组标本的超微结构并加以比较。结果高度恶性和低度恶性胶质瘤超微结构发生一系列改造和重建,都存在不同程度的微血管形成,尤以血脑屏障的改变为著。高级别胶质瘤超微结构的镜下改变更为明显,主要表现为基膜不同程度的增厚,细胞外基质以及胶质膜的改变,微血管形成增多。结论不同级别胶质瘤的超微结构均有所改变,形成的微血管可以为肿瘤细胞提供营养,高级别胶质瘤较低级别胶质瘤血脑屏障基膜的完整性有局部破坏,细胞外基质、胶质膜的改变以及窗孔结构形成等可能是胶质瘤高侵袭性的超微结构基础。
Objective To research the ultra-structure basic of infestation in brain glioma.Methods Classify forty-five patients with brain glioma proved by surgery and pathology were divided into two groups :malignancy group and low grade malignancy group .Five spec-imens of normal brain from craniocerebral trauma decompression operation were collected as the control object of observation .Production and application of transmission electron microscope of three groups were observed respectively and the ultrastructure of specimens was compared . Results A series of transformation and reconstruction occurred in both high grade and low grade gliomas ultrastructure ,there were different levels of angiogenesis and blood brain barrier changes .The changes of high grade gliomas ultrastructural under microscope was more obvious , mainly for the different degree of thickening of basement membrane ,extracellular matrix as well as the glial membrane changes ,and more mi-crovessel formation.Conclusion The ultrastructure of different grade gliomas are changed ,the formation of the microvascular can supplying nutrients to the tumour cells.The blood brain barrier membrane integrity of local damage can be found in high grade gliomas more obvious than low-grade glioma.The changes of extracellular matrix and glial membrane as well as the window hole structure formation may be the ul -trastructural basis of high invasiveness of glioma .
