摘要
目的:探讨彩色多普勒超声(CDUS)和超声造影(CEUS)在青春后期及成人睾丸扭转诊断中的价值。方法:使用彩色多普勒超声和超声造影观察12例18~66岁临床疑诊为睾丸扭转的患者。结果:12例患者中,11例为睾丸扭转,1例为附睾头囊肿压迫精索导致的睾丸淤血。以患侧睾丸血供减少或消失作为诊断标准,彩色多普勒超声诊断睾丸扭转的灵敏度为100%,准确率为91.7%。以超声造影显示睾丸无灌注、灌注缺损或"晕圈征"为诊断标准,超声造影的灵敏度和准确率皆达100%。结论:彩色多普勒超声和超声造影都是诊断青春后期及成人睾丸扭转的良好成像手段,超声造影的准确率可能相对更高,但尚需要大样本的后续研究来证实其在诊断睾丸扭转中的价值。
Objective:To evaluate the value of color Doppler ultrasonography(CDUS) and contrast-enhanced ultrasonography(CEUS) in diagnosing testicular torsion in late adolescence and adult.Methods:Twelve patients aged 18 to 66 years with suspected testicular torsion received both CDUS and CEUS.Results:Of the 12 patients,11 patients had testicular torsion and 1 patient presented testicular congestion secondary to spermatic cord compression by an epididymal cyst.On CDUS,the basis for diagnosing testicular torsion was the absence or decrease of flow in the symptomatic testis,which yielded a sensitivity of 100% and an accuracy of 91.7%.Non-perfusion,perfusion defect or the 'ring sign' displayed on CEUS had a sensitivity of 100% and an accuracy of 100% in the diagnosis of testicular torsion.Conclusions:The results indicate that both CDUS and CEUS are useful for the diagnosis of testicular torsion in late adolescence and adult,and CEUS has a higher accuracy.However,further studies are needed to confirm the value of CDUS and CEUS for the diagnosis of testicular torsion.
