磁共振、核素扫描对非创伤性股骨头坏死的发病机制研究 被引量:2

摘要 目的通过对非创伤性股骨头坏死的核素扫描和磁共振影像资料的分析,探讨其发病机制。方法采用回顾性研究方法,从门诊和住院患者中随机选取60例非创伤性股骨头坏死的患者,其中门诊10例患者采用核素扫描(single photon emission computed tomography,SPECT),50例住院患者采用磁共振(magnetic resonance imaging,MRI)扫描,收集影像资料加以总结分析。结果总结10例非创伤性股骨头坏死的SPECT表现,其中14个髋关节考虑股骨头坏死,14个髋关节显示坏死区为浓聚表现。住院的50例非创伤性股骨头坏死患者中接受MRI检查,其中82髋关节显示长T2信号。长T2信号是股骨头内充血水肿的典型表现。总结以上结果显示,非创伤性股骨头坏死为充血性疾病,而非缺血性疾病。结论股骨头坏死是最初是因为股骨头充血内水肿,骨内压力增高,静脉及静脉窦壁比动脉壁薄,所以最早受影响,静脉回流受阻,引起股骨头内压力增高,这是一个恶性循环的过程。最终导致股骨头坏死。这与静脉瘀滞学说相符。这将为股骨头坏死的治疗提供帮助。
