经尿道前列腺剜除术13例报告

The Report of Transurethral Prostate Enucleation for the 13 Cases

目的:探讨经尿道前列腺剜除术治疗良性前列腺增生症在基层医院的应用。方法:通过前列腺剜除术术中结合镜鞘与电切袢的运用,找到增生腺体和外科包膜之间的界面,将增生腺体自外科包膜内剥离、切碎、冲出,对术中情况及预后效果进行分析。结果:切除组织30～75 g,手术时间60～170 min,平均110 min。术中出血量40～400 ml,术后冲洗时间平均84 h,术后未发生短期尿道刺激症、膀胱颈挛缩、尿道狭窄病例。本组病例无水中毒及循环负荷过重,术后4～10 d后拔出尿管。有轻微尿失禁1例。随访期间13例患者中无死亡病例,无再次手术及永久性尿失禁等并发症发生。结论:前列腺剜除术出血少,速度快,效果好,并发症少,安全性高,值得基层医院推广应用。

Objective：To enucleation of the prostate for benign prostatic hyperplasia in grass-roots hospital applications.Method：By combination of sheath in enucleation of the prostate and transurethral application of loop,find coated interface between proliferative glands and surgical,gland hyperplasia surgery coated peel,shredded,washed out,effect and prognosis analysis of operation situation.Result：Subtotal organization 30-75 g,60-170 min operation time,average 110 min.Intraoperative blood loss 40-400 ml,after flushing time average 84 h,with short urethra stimulation did not occur after surgery,urethral stricture,bladder neck contracture cases.This group case was no overloading and circulating water poisoning,remove urinary catheter after 4-10 days after surgery.Slight urinary incontinence 1 cases.Theve was no death case during the follow-up,there was no permanent incontinence and other complications of reoperation-free in 13 cases. Conclusion：Enucleation is less bleeding,fast,effective,less complications,high security,worthy of popularization and application of grass-roots hospitals.
