摘要
目的:探讨高血压患者颈动脉内膜中层厚度与踝臂指数(ABI)的相关性。方法:选择住院高血压患者,分别测量每例患者的ABI、颈动脉内膜中层厚度(IMT)、血压、体质量、血糖、胆固醇、血尿酸、血肌酐。根据颈动脉内膜中层厚度(IMT)分为颈动脉粥样硬化组(AS组)和非颈动脉粥样硬化组(非AS组),比较两组的ABI变化;同时对AS组的ABI与IMT进行相关性分析。结果:共入选98例,男60例,女38例,平均年龄69.5岁。检出IMT异常51例,ABI异常31例。AS组有22例(43%)ABI异常,非AS组有9例(19%)ABI异常(P<0.05)。AS组的ABI为(0.78±0.07),非AS组为(0.89±0.18),差异有统计学意(P<0.05)。在AS组中,ABI减低患者的IMT较ABI正常患者明显增厚分别为(1.21±0.17)和(1.00±0.09),差异有统计学意义(P<0.05)。经直线相关性分析显示:高血压与颈动脉粥样硬化的严重程度呈正相关(r=0.44,P<0.05);踝臂指数与颈动脉粥样硬化严重程度呈负相关(r=-0.39,P<0.05)。结论:高血压是颈动脉粥样硬化的重要危险因素,高血压患者常规测量ABI对早期发现动脉粥样硬化有重要价值。
Objective Discussing the correlation in hypertension with ankle brachial index (ABI) and carotid artery intima - media thickness. Methods I selected more than one patient with hypertension in our hospital. I measured each patient with ABI, carotid artery in- tima -media thickness (IMT), blood pressure, body weight, blood glucose, cholesterol, blood uric acid and blood creatinine. On the ba- sis of IMT, I divided into two groups - Carotid atherosclerosis group (AS) and non Carotid atherosclerosis group ( non AS) , compared with ABI in two groups. At the same time,I analysed the correlation with ABI and IMT in AS group. Results 98 cases were selected, male 60 cases, female 38 cases, aged 69. 5 years old. 51 abnormal cases were found for IMT,31 abnormal cases were found for ABI. 22 abnormal ABI cases(43% ) were found in AS group. 9 abnormal ABI cases( 19% ) were found in non AS group(P 〈0. 05). ABI in AS group: 0. 78 ±0.07, ABI in non AS group: 0. 89 ±0. 18, (P 〈0.05). In AS group, ABI patient with impaired IMT than in normal ABI patients in- creased significantly, ( 1.21 ±0. 17) and( 1.00 ±0. 09), ( P 〈0. 05). Linear correlation showed: hypertension and carotid atherosclerosis severity are positive correlation ( r = 0.44, P 〈 0. 05 ) ; ankle brachial index and carotid atherosclerosis severity are negative correlation ( r =0. 39 ,P 〈 0. 05 ). Conclusion Hypertension is a important risk factors of carotid atherosclerosis. Measurement of ABI in hypertensive pa- tients have an important value for the early detection of atherosclerosis.
