摘要 目的探讨MMP-2、MMP-9在葡萄胎组织中的表达情况,以确定二者预测葡萄胎恶变的可能。方法收集41例清宫术后病理诊断为葡萄胎患者的首次清宫组织,并收集17例正常早孕(<12周)无其他疾病、要求终止妊娠的绒毛组织作为对照组。将组织分为正常绒毛组(17例)、葡萄胎组(29例)、侵葡组(12例),观察MMP-2、MMP-9在侵蚀性葡萄胎、葡萄胎以及正常绒毛表达强度。结果 MMP-2、MMP-9在侵葡组、葡萄胎组以及正常绒毛组的灰密度值依次为70.93±8.87/86.91±8.81,102.40±7.13/109.38±5.60,105.64±5.30/111.27±4.33,侵葡组MMP-2,MMP-9的阳性表达明显高于其他2组(P均<0.01),但葡萄胎组与正常绒毛组相比无显著性差异(P>0.05);在侵葡组的表达中MMP-2和MMP-9呈正相关。结论 MMP-2、MMP-9高表达可能在葡萄胎、侵蚀性葡萄胎发生发展过程中有促进作用,能促进葡萄胎组织的恶变。在含有高危因素的葡萄胎恶变患者中,MMP-2、MMP-9的表达明显升高,联合检测MMP-2、MMP-9对预测葡萄胎的恶变具有一定的临床意义,有可能成为临床预测葡萄胎恶变的重要指标。 Objective It is to observe the expression of MMP- 2 and MMP -9 in hydatidiform mole, and approach the feasibility to predict the malignant change of hydatidiform mole. Methods 41 cases of curettage tissues form patients with hyda- tidiform mole by pathological confirmed and 17 cases of villus tissues from normal pregnancy ( 〈 12 weeks) without other dis- eases and to terminate the pregnancy as control were collected. The organizations were divided into normal villi group ( 17 ca- ses) , hydatidiform mole group (29 cases) , and invasive hydatidiform mole (12 cases). The expression intensity of MMP -2, MMP - 9 in invasive mole, mole and normal villi were observed. Results The gray average density value of MMP - 2, MMP - 9 in invasive mole group, mole group and normal villi group were 70.93± 8.87/86.91 ± 8.81, 102.40± 7.13/109.38 ± 5.60, 105.64 ±5.30/111.27 ± 4.33, the positive expression of MMP - 2 and MMP - 9 in invasive mole was markedly higher than the others ( all P 〈 0.05 ) , bu there was no significant difference between mile group and normal villi group ( P 〉 0.05 ) ; there was significant positive correlation in the expression of MMP - 2 and MMP - 9. Conclusion High expression of MMP - 2 and MMP - 9 may do auxo-action in the development process of hydatidiform mole and invaslve mole, and promote the malig- nant transformation of hydatidiform mole. For the high risk factors patients with malignant change, the expression of MMP -2 and MMP -9 are obviously rose, combined detection of MMP -2 and MMP -9 in predicting malignant transformation in hyda- tidiform mole is of clinical significance, has the poteniial to he clinically important predictor of hydatidiform mole malignant change.
