摘要 目的:归纳Glut-1及其调控因子HIF-1α与P53在卵巢癌组织中免疫组化染色阳性表达的的规律,探讨其与临床病理参数的相关性,分析Glut-1在诊断中应用的价值并推测缺氧在卵巢癌发病中的作用。方法:免疫组织化学EnVision法检测Glut-1、HIF-1α及P53在63例卵巢癌组织中的表达,采用SPSS统计分析软件进行相关性分析。结果:在卵巢癌组织中,(1)Glut-1呈弥漫性、局灶性或散在性多种分布模式,且随染色评分的增加,其在细胞膜上的表达愈发突出,并与HIF-1α阳性表达区域部分重叠。(2)Glut-1的表达与HIF-1α、P53有相关性,呈序列正相关。(3)Glut-1表达与组织学分级、Kurman分型均呈序列正相关,与FIGO分期无相关性;P53表达仅与Kurman分型有相关性,与组织学分级、FIGO分期无相关性;HIF-1α则与组织学分级、Kurman分型及FIGO分期均无相关性。(4)卵巢低级别与高级别浆液性癌中Glut-1与P53的表达均有显著不同,且Glut-1的表达强度与卵巢浆液性癌的分级呈正相关,而HIF-1α的表达与分级无关。结论:不同亚型的卵巢癌,其微环境缺氧状态不同,Glut-1与P53联合检测是有助于指导卵巢癌分型及浆液性卵巢癌分级的指标。 Objective: To summarize the expression of Glut-1 and its regulation factors HIF-I~& P53 immunohistochemical staining in o- varian cancer, investigate its relationship with the clinical pathological parameters, analyze the effect of Glut-1 in pathological diagnosis. and thus speculate the role of hypoxia in ovarian cancer. Methods: A total of 63 ovarian carcinomas were examined with Glut-I,HIF-! and P53 immunohistochemical staining. Using SPSS 16.0 for windows correlation analysis was applied. Results: In ovarian eancer cells, (1) Glut-1 showed diffuse, focal or scattered distribution patterns. As the staining score increased, the cell membrane beeame more stained. Positive tumor cells for Glut-1 were found contained in areas positive for HIF-lα. (2)Glut-1 expression showed positive correlations with HIF-lα and P53 staining. (3)It also displayed positive correlations between Glut-I expression and histological grade and Kurman subtype, but didn't exist in FIGO stage; P53 was correlated with Kurman subtype, but it not significant in histological grade and FIGO stage; the ex- pression of HIF-lc~ showed no correlation with histological grade, Kurman subtype and FIGO stage. (4)Expressions of Glut-1 and P53 were significantly different in ovarian low grade and high grade serous carcinoma. The staining intensity of Glut-1 was increasingly positively correlated with grading of serous carcinomas, while HIF-1α not. Conclusion: For different Kurman subtypes of ovarian cancer, the hypoxic status of microenvironment are different. Glut-1 and P53 joint detection is extremely efficient to divide ovarian cancer and serous carcinoma.
