目的:分析我院静脉用药集中调配中心审方过程中发现的各种不合理用药情况,以期为临床合理用药提供参考。方法:采用回顾性分析方法,对2010年1月-2012年12月我院PIVAS全部医嘱中不合理医嘱进行归纳、整理,对拒配的原因进行分析。结果:不合理用药288例情况基本可以分为给药浓度不当、溶剂选择错误、药物之间配伍不当、超剂量给药、给药频次不当、给药浓度不当等。结论:通过药师审核医嘱,可及时纠正不合理理用药,保障患者合理用药。

Objective：To promote rational clinical medication and ensure patients medication safety. Methods： To summarize various un- reasonable Drescriptions occurred in PIVAS of our hospital from January, 2010. to December, 2012. Then to analyze the result. Result：288 pieces of unreasonable prescriptions were found, the causes of the unreasonable prescriptions lie in improper administration, unreasonable choice,improper combination ,over-dosage, improper administration frequency and incorrect drug concentration. Conclusion： The rational use of drugs level can be effectively enhanced through pharmacists applying their the pharmaceutical expertise to the chegldng of medical or-ders.
