随机尿微量白蛋白与尿肌酐比值对慢性心力衰竭患者的诊断价值 被引量:8

摘要 目的通过对慢性心力衰竭(CHF)患者与健康对照者随机尿液中微量白蛋白与肌酐比值(UACR)水平的比较,探讨UACR与CHF的关系。方法选择心血管内科患者517例,按糖尿病(DM)有无分为糖尿病组和非糖尿病组。检测氮末端B型钠尿肽前体(NT-proBNP)确定心衰患者178例,并按心功能分级分为Ⅱ级组49例,Ⅲ级组71例,Ⅳ级组58例。分别检测心衰患者的尿微量白蛋白(mALB)、尿肌酐(Cr)、红细胞分布宽度(RDW),分析不同分组之间UACR的差异。结果 CHF患者尿液UACR与健康对照组比较有显著差异(P<0.01)。不论DM有无,UACR均与心衰正相关(P<0.01)。按心功能分级分组后组间差异显著(P<0.01)。UACR与RDW、NT-proBNP对心衰诊断呈正相关(R=0.314;R=0.357,P<0.01)。结论尿液UACR在CHF患者中明显升高,与是否有DM无关,且升高幅度与病情的严重性相一致,联合检测UACR与RDW、NT-proBNP对CHF诊断有重要意义。
