摘要
目的探讨超声诊断婴儿单侧肺动脉异常起源的价值。资料与方法回顾性分析经超声诊断并经多层螺旋CT或外科手术证实的5例单侧肺动脉异常起源的患儿的超声心动图,总结单侧肺动脉异常起源的超声特点。结果 5例患儿中,右肺动脉异常起源于升主动脉3例,肺动脉悬带1例,右肺动脉缺如1例。其超声表现特征为:肺动脉远端无分叉,肺动脉主干仅与左支延续,右肺动脉起源于升主动脉则为右肺动脉异常起源于升主动脉;多切面反复观察均无法探及另一侧肺动脉,则为右(左)肺动脉缺如;左肺动脉起源于右肺动脉的中远段,则为肺动脉悬带。结论超声具有便捷、无创、可重复及可以发现其他先天心脏畸形的优点,可以诊断婴儿单侧肺动脉异常起源,有助于早期临床确诊和手术治疗,应作为诊断该病的首选方法。
Purpose To explore the pulmonary artery anomalous origi value of ultrasound in diagnosing unilateral n of the infants. Materials and Methods Echocardiography of five infants with unilateral pulmonary artery anomalous origin diagnosed by ultrasound and confirmed with multislice spiral CT (MSCT) or surgery were retrospectively analyzed, and ultrasound characteristics of unilateral pulmonary artery anomalous origin were summarized. Results In all the five infants, three cases appeared as anomalous origin of the right pulmonary artery from the ascending aorta: one case appeared as pulmonary artery sling and one case as right puhnonary artery absence. Ultrasound characteristics were: no bifurcation in distal puhnonary artery, main trunk of pulmonary artery was only continuous to its left branch, right pulmonary artery originated in the ascending aorta was defined as anomalous origin of the right pulmonary artery from the ascending aorta: unable to detect the contralateral pulmonary artery with repeated multi-slice observation was defined as right (left) pulmonary artery absence: left pulmonary artery originated from middle or distal segment of right pulmonary artery was defined as pulmonary artery sling. Conclusion Ultrasound has the advantages of convenience, non-invasiveness, repeatability and the capability of detecting the other concurrent congenital heart abnormalities, is able to diagnose unilateral pulmonary artery anomalous origin in infants, thus will contribute to the early clinical diagnosis and surgical treatment and should be used as the preferred diagnosis method of this disease.
