摘要
患者女,21岁。因颈部淋巴结肿大,自行口服网上所购中药"七络丹"8个月,双手足丘疹、颜面颈部躯干色素沉着5个月。查体:双手足见多个密集分布的角化性丘疹,约小米粒大小;颜面、颈部、躯干见大片棕褐色色素沉着,界不清。皮损组织病理检查示:表皮角化过度,真皮浅层慢性炎症细胞浸润和小血管扩张。尿砷22.10μmol/L。诊断:中药致慢性砷中毒。经硫代硫酸钠治疗后,丘疹和色素沉着明显减退。
A 21-year-old girl had swollen lymph nodes on neck and had taken formulated herbal medicine for 8 months. She had papules on the both hands and feet, and hyperpigmentation on the face, neck and trunk for 5 months. Physical examination indicated dense millet-sized keratotic papules on the both hands and feet. Hy- perpigmentation with undefined borders could be seen on the face, neck and trunk. The histopathological ex- amination showed epidermal hyperkeratosis, dermal inflammatm3~ cell infiltration and dilated blood vessels. The arsenic concentration in urine was 22.10pmml/L. The patient was diagnosed with chronic arsenic poi- soning caused by Chinese medicine. The hyperpigmentation and papules were improved following sodium thi- osulfate treatment.
