功能影像学诊断弥漫性轴索损伤的研究进展 被引量:6

摘要 对于交通事故导致的创伤性脑损伤(traumatic brain injury,TBI),CT及常规MRI检查显示的损伤情况不能充分解释临床症状.其中一种可能的解释就是这些患者存在弥漫性轴索损伤(diffuse ax-onal injury,DAI) [1-3].Adams等[4]在1982年将DAI作为一种独立的疾病提出来.DAI是指头部受到外力作用后使颅脑产生旋转加速度,即由于外伤后脑组织内部易发生剪应力作用,导致神经轴索和小血管损伤.CT及常规MRI检查对DAI不敏感,无法显示脑白质纤维束损伤及细小针尖样小出血灶.目前,功能MRI成像方法如磁共振弥散张量成像(diffusion tensor imaging,DTI)、磁敏感加权成像(susceptibility weighted imaging,SWI)、磁共振波谱(magnetic resonance spectroscopy,MRS)可以显示在CT及常规MRI上无法显示的病变,明显提高DAI的检出率.因此,笔者就较为常见的几种功能MRI成像技术诊断DAI进行综述.
