摘要
目的探讨老年胃癌患者手术后呼吸功能锻炼改善肺功能的疗效,以及其对手术后肺部感染的预防作用。方法将180例老年胃癌患者随机分为两组,各90例;观察组采取呼吸功能锻炼的方法,而对照组不采取呼吸功能锻炼,比较两组患者手术后肺部感染的发生率和其他并发症的情况。结果观察组肺部感染率为14.4%,对照组为40.0%,两组比较,差异有统计学意义(P<0.05);观察组使用抗菌药物天数和住院天数分别为(4.3±1.4)d和(9.4±1.5)d,对照组分别为(7.4±1.2)d和(14.6±1.3)d,两组相比,差异有统计学意义(P<0.05);观察组手术后动脉血氧分压、二氧化碳分压以及指末氧饱和度分别为(10.41±0.91)kPa、(6.09±0.85)kPa、(96.20±1.14)%,对照组分别为(9.89±0.05)kPa、(6.80±0.12)kPa、(90.20±0.40)%,两组比较,差异也有统计学意义(P<0.05)。结论呼吸功能锻炼能显著减少老年胃癌患者术后肺部并发症,改善患者肺功能。
OBJECTIVE To explore the effect of respiratory function exercise on the improvement of lung function as well as the prevention of postoperative pulmonary infections in elderly patients with gastric cancer. METHODS A total of 180 elderly patients with gastric cancer were randomly divided into two groups, with 90 cases in each; the observation group adopted the method of respiratory function exercise, while the control group did not, then the incidence of postoperative pulmonary infections and other complications was compared between the two groups. RESULTS The incidence of pulmonary infections of the observation group was 14.4%, compared with 40.0% of the control group, the difference was significant (P〈0. 05) ; the duration of use of antibiotics was (4.3±1. 4) days in the observation group, (7.4±1.2) days in the control group; the length of hospitalization was (9.4±1.5) days in the observation group, (14.6±1.3)days in the control group, the difference was significant (P〈0.05). The postoperative partial pressure of arterial oxygen was (10. 41 ± 0. 91)kPa in the observation group, (9.89±0.05)kPa in the control group; the postoperative partial pressure of carbon dioxide was (6.09 ±0.85)kPa in the observation group, (6.80±0. 12)kPa in the control group; the oxygen saturation was (96.20 ±1.14)% in the observation group, (90.20±0.40)M in the control group, the difference was significant (P〈 0.05). CONCLUSION Respiratory function exercise can significantly reduce postoperative pulmonary complications, improve lung function.
