乳腺浸润性导管癌患者人乳头状瘤病毒感染的分析

摘要 目的通过观察乳腺浸润性导管癌不同病理人乳头状瘤病毒(HPV)高危型HPV18、HPV16感染情况,探讨HPV18、HPV16感染与乳腺浸润性导管癌生成及转化的相关性。方法随机抽取2007年1月-2012年1月128例女性乳腺癌患者送检组织标本,将其分为观察组98例和对照组30例,观察组为乳腺浸润性导管癌患者,对照组为正常乳腺组织;观赛组采用原位杂交方法检测HPV18、HPV16表达情况。结果浸润性导管癌组织中HPV16、HPV18与HPV16和HPV18三者阳性表达率分别为66.33%、71.43%和52.04%,正常乳腺上皮组织中阳性表达率为16.67%、23.33%和10.00%,差异有统计学意义(P<0.05);浸润性导管癌Ⅰ期HPV16、HPV18分子和HPV16与HPV18分子三者阳性表达率分别为43.48%、47.83%和17.39%;Ⅱ期分别为60.00%、63.33%和50.00%;Ⅲ期分别为77.42%、83.87%和67.74%;Ⅳ期分别为92.86%、100.00%和78.57%;不同病理分期间的患者体内三类病毒阳性表达率与Ⅰ期相比,差异有统计学意义(P<0.05),随着癌症病情的恶化,HPV18、HPV16阳性表达增强。结论 HPV18或HPV16感染与乳腺浸润性导管癌的生成和转化存在相关性,且随着乳腺癌病情恶化,HPV18、HPV16表达增强,此发现可为治疗乳腺癌患者提供一种新治疗途径。 OBJECTIVE To observe the status of different pathological high-risk human papilloma virus(HPV) HPV 18 and HPV 16 infections in patients with breast invasive ductaI carcinoma and explore the correlation between the HPV18 and HPV16 infections and the formation and transformation of invasive ductal breast cancer so as to provide a theoretical basis for the diagnosis and treatment of breast cancer. METHODS The submitted specimens of 128 cases of female breast cancer patient were randomly collected from Jan 2007 to Jan 2012, and were di vided into the observation group (98 cases) and the control group (30 cases), the patients with breast invasive ductal carcinoma were assigned in the observation group, the infiltrating ductal cancer patients with normal breast tissues as the control group. The expression of HPV18 and HPV16 was detected by in situ hybridization method. RESULTS The positive rates of expression of the HPV18 molecules, HPV16 molecules, and HPV18 and HPV16 molecules were respectively 66.33%, 71. 43% and 52. 04% in the infiltrating ductal carcinoma tissues, which were respectively 16. 67%,23.33M, and 10.00% in the normal breast epithelial tissue, the difference was significant (P〈0.05) ; In addition, the positive expression rates of the three types of viruses in the patients with invasive ductal carcinoma were respectively 43. 48%, 47.83% and 17.39% in the stage I , 60.00%, 63.33% and 50.00% in the stage II , 77.42%, 83.87% and 67.74% in the stage III , 92. 86%0,100.00% and 78.57% in the stage IV. The difference in the positive expression rate of the three kinds of viruses between the stage I and other three stages was significant (P〈0.05). With the deterioration of the condition of cancer, the expression rate of HPV18 and HPV16 molecules was increased, and there was a positive correlation between the transformation and the expression of HPV18 and HPV16 molecules (r = 0.6834, P〈0.05). CONCLUSION There is a correlation between the HPV18 and HPV16 infections and the formation and transformation of breast infiltrating ductal carcinoma; along with the deterioration of breast cancer, the expression rate of HPV18 and HPV16 is increased, the finding can provide a new therapeutic approach for the treatment of breast cancer.
