糖尿病足感染不同深度的临床评估

Clinical manifestations of patients with different severities of diabetic foot infections

目的观察糖尿病下肢溃疡的不同分期及伴随不同程度感染的临床表现,探究糖尿病足的综合护理方法,并对其干预效果进行评价。方法将医院2011-2012年收治的120例糖尿病足感染患者随机分为试验组和对照组,对两组患者进行临床分期及对症治疗;对照组接受常规护理,试验组接受综合护理干预。结果经过治疗及护理后,两组患者溃疡面均有不同程度好转,0级人数试验组35例占58.3%,对照组26例占43.3%,试验组人数显著高于对照组(P<0.05),溃疡较严重(3～5级)患者,试验组4例占6.7%,对照组9例占15.0%,两组患者感染率均有下降,试验组下降幅度36.7%,显著高于对照组的20.0%(P<0.05);试验组中重度感染(4～5级)患者2例占3.4%,显著少于对照组的5例占8.3%(P<0.05);试验组总有效率93.3%,对照组85.0%,试验组总有效率显著高于对照组(P<0.05)。结论综合护理能控制全身状况,改善局部微循环,提高患者创面愈合水平及降低感染水平,显著提高患者治疗效果。

OBJECTIVE To observe the clinical manifestations of the patients with diabetic foot infections and explore the synthetic nursing process of lower limbs ulcer surface so as to evaluate the intervention effectiveness. METHODS A total of 120 diabetic patients who were treated in the hospital from 2011 to 2012 were randomly divided into the experimental group and the control group. Both groups accepted the same treatment and normal nursing, while the experimental group was treated with synthetic nursing intervention. RESULTS After the treat- ment and nursing, both groups developed but the experimental group had 35 （58.3 %） of degree 0,while the control group had 26 （43.3%）. The experimental group had 4（6.7%） of severe ulcer（degree 3-5）, while the control group had 9（15 %）. The experimental group had more degree 0 patients and less severe ulcer than control group （P〈0.05）. The infection was controlled with a decreasing infection rate. The experimental group had a larger de scend extent 36.7% in infection rate than control group of 20.0% （P〈0.05）. And experimental group had less severe infection of 2（3.3%） than control group of 5（8.3%）（P〈0.05）. The experimental group had a higher total valid rate of 93.3% than the control group of 85.0% （P〈0.05）. CONCLUSION The synthetic nursing can help control the systemic condition, improve the local microcirculation and wound healing, reduce the incidence of infection, and significantly improve the therapeutic effect.
