多焦视网膜电图评价视网膜脱离的视网膜功能 被引量:10

The evaluation of the visual function in retinal detachment using multifocal electroretinography

摘要 目的 探讨多焦视网膜电图 (multifocal electroretinography,MERG)对视网膜脱离 (retinadetachment,RD)的视功能客观评价意义。 方法 应用 VERIS4.0视诱发反应图像系统检测 2 1例 RD患者的 2 2只患眼和 36例正常人的 42只眼的 MERG。测试野的水平视角为± 2 6 .6°,垂直视角为± 2 2 .1°,采用Burian- Allen接触镜电极 ,在 8min(分 16段 )记录 10 3个视网膜部位的反应。 结果  RD眼于 4个象限、黄斑区和黄斑外区 MERG a波、b波潜伏期及 b波振幅密度、振幅之和均明显低于正常对照组 (P<0 .0 1) ;脱离区、非脱离区和正常眼的 MERG b波振幅密度分别为 (3.44± 2 .85 )、(6 .34± 3.31)、(2 1.32± 6 .48)n V/ deg2 ,三者之间的差异均有非常显著性意义 (P<0 .0 5 )。 结论 多焦 ERG能客观定量评价 RD的黄斑部、后极部的脱离区和非脱离区的视功能。 Objective [WT5”BZ]To explore significance of multifocal ERG(MERG) in evaluating visual function in patients with retinal detachment(RD). Methods MERG was tested using VERIS 4.0 visual evoked response imaging system for 21 patients with retinal detachment (22 eyes) and 36 normal subjects (42 eyes). The stimulative visual angles subtended ±26.6° horizontally and ±22.1° vertically. Burian Allen contact lens electrode was used to record the responses from 103 retinal locations in 8 min (16 segments). Results The latencies of a wave and b wave at the 4 quadrans, macular area and extra macular area in RD group were significantly delayed and their amplitudes markedly decreased comparing with normal group (P<0.01). The amplitude densities of b wave at detached area, attached area in RD eyes and at the retina in normal eyes were (3.44±2.85),(6.34±3.31),(21.32±6.48) nV/deg 2 respectively. There was statistically significant difference among them (P<0.05). Conclusion MERG can evaluate objectively and quantitatively the visual function at macular area, posterior polar detached area and attached area in RD.
