持续植物状态患者胃造瘘与鼻饲营养疗效比较 被引量:1

Comparison of percutaneous endoscopic gastrostomy and nasogastric feeding in patients with persistent vegetative state

摘要 目的探讨持续植物状态(PVS)患者胃造瘘与鼻饲肠内营养的营养状况改善及并发症的发生情况。方法将PVS患者53例,分为经皮内镜胃造瘘术(PEG)组24例,和鼻饲组(NGF)29例,给予相同的营养液,采用传统的营养指标作为判断标准,对两组患者的营养状况进行评定,并对两组患者肺部感染、消化道出血、顽固性呃逆、低蛋白血症或电解质紊乱等并发症进行比较,最后对两组患者行PVS评定。结果 PEG组患者的营养指标均高于NEG组,肺部感染、消化道出血、低蛋白血症及电解质紊乱发生率较NEG组低,差异有显著性。PEG组PVS评分亦高于NEG组。结论PEG肠内营养支持使PVS患者营养状况得到了改善。减少了并发症,有利于PVS意识状态的恢复,优于NGF。 【Objective】To compare the enteral nutrition efficiency and complication of percutaneous endoscopic gastrostomy(PEG) with nasogastric feeding(NGF) in patients with persistent vegetative state(PVS).【Methods】 Fifty-three patients with persistent vegetative state were divided into two groups: PEG enteral nutrition group(n=24) and NGF group(n =29).Patients in both groups were fed with same nutrient fluids.Orthodox nutrition indexes were used to to evaluate the nutritional status of the two groups.Complications such as pulmonary infections,gastrointestinal hemorrhage,intractable hiccup,hypoalbuminemia and electrolyte disturbances were also compared.At last the PVS scores were compared between the two groups.【Results】Orthodox nutrition indexes of PEG group were higher than NGF's.The incidences of all complications in PEG group were significantly lower than those in NGF group(all P 0.05) respectively.The PEG group's PVS scores were higher than NGF's scores.【Conclusions】PEG enteral nutrition had an advantage over NFG in improving status of patients and in reducing complications and recovering of conscious state of PVS.
