食管静脉曲张套扎后近期再出血急诊二次套扎可行性分析

The feasibility of emergency secondary esophageal variceal ligation (EVL) for recent re-bleeding after EVL

目的研究食管静脉曲张套扎(EVL)后近期再出血急诊二次套扎可行性。方法食管静脉曲张行套扎387例(多为单纯食管静脉曲张)出现套扎后出血35例(套扎后2周内发生),男27例,女8例,其中33例均急诊行胃镜检查。结果 在33例患者中,有24例由食管静脉曲张导致,并对其行6连发食管静脉曲张套环结扎术,有5例由门脉高压性胃病或消化性溃疡导致,并给予镜下止血治疗,其余4例无法明确出血原因。结论所有行食管静脉曲张二次套扎的患者均血止,证明食管静脉曲张套扎后近期再出血急诊二次套扎具有可行性。

【Objective】To study the probability of emergency secondary EVL for early stage hemorrhage after EVL.【Methods】Esophageal varices in 387 patients was treated by endoscopic variceal ligation（the most patients were simple esophageal varices）,early stage hemorrhage in 35 patients was treated by EVL（in two weeks）,male 27 cases,female 8 cases.There were 33 patients who needed emergency gastroscopy,the other two died.【Results】 During above 33 patients,there were 35 patients caused by Esophageal varices,and performed EVL（MBL-6-F） on them,there were 5 patients caused by portal hypertensive gastropathy or pepticulcer,and received the endoscopic treatments,the reason for the other 4 patients was unknown.【Conclusions】All the patients who did the second EVL stopped the bleeding,so that emergency secondary EVL for early stage hemorrhage after EVL is feasible.
