摘要
目的观察丙泊酚对静息状态及预收缩人体肺内动脉张力的影响,探讨其作用是否依赖内皮功能。方法选取24例患者肺组织标本,随机分为三组:血管内皮完整组(N组)、机械去血管内皮组(D组)及左旋硝基精氨酸甲酯(L-NAME)抑制内皮功能组(L组)。分离人体肺内动脉,采用微血管张力测定技术,分别测定三组人体肺内动脉静息及预收缩后舒张率。结果三组静息状态下人体肺内动脉舒张率差异无统计学意义,而呈浓度依赖性舒张由U46619预收缩的血管环(N组EC50=206.7μmol/L,D组EC50=152.8μmol/L,L组EC50=331.0μmol/L),L组Emax和内皮舒张率明显低于N组和D组(P<0.05),二者呈正相关(r=0.872,P<0.01)。结论丙泊酚能舒张人肺内动脉,且内皮影响其舒张效应。
Objective To observe the effect of propofol on human intrapulmonary arterial rings either under basal vascular tones or precontracted by U46619 and to investigate the correlation between this effect and vascular endotheliurru Methods A total of twenty-four imrapulmonary arterial rings from 24 patients were randomized into three groups= group N with intact endothelium, group D with denuded endothelium and group L with L-NAME preconditioning. Human intrapulmonary arterial rings were isolated and the effect of propofot on human intrapulmonary arterial rings in three different groups was measured by microvascular tension measurement technique. Results Propofol has no effect on the intrapulmonary arterial rings under basal vascular tones among three groups. However, propofol relaxed all intrapulmonary arterial rings precontraced by U46619 in a dose dependent manner (EC50 in group N= 206.7 μmol/L, EC50 in group D= 152.8 μmol/L, EC50 in group N=331.0 μmol/L). Compared with group N and group D respectively, the relaxation rate in group L showed significant difference(P〈00 05), which was correlated with the relaxation rate mediated by endothelium (r= 0. 872,P〈0.01). Conclusion Propofol may relax intrapulmonary arterial rings in a dose-dependent way, and perhaps, the endothelium may involve in this effect.
