康复医学科超长住院日的影响因素分析

Analysis of influence factors of long length-of-stay in the department of rehabilitation medicine

目的探讨某三级综合医院康复医学科超长住院日的影响因素，为有效缩短该科平均住院日提供科学依据。方法通过病案管理与医疗统计系统，提取2007--2011年康复医学科出院患者病案首页数据，将住院天数大于30天定义为超长住院日；利用logistic回归模型分析该科超长住院日的影响因素。结果多因素logistic回归分析显示转科、神经系统疾病及其后遗症、院内感染、有合并症、术前住院天数均是影响康复医学科超长住院日的影响因素（P〈0．05）。结论康复医学科住院日受多种因素的影响，应针对相关影响因素采取合理措施，控制住院日。

Objective To explore the influence factors of long length-of stay in the department of rehabilitation medi- cine. Methods The first page of medical record of discharged patients in the department of rehabilitation medicine from 2007 to 2011 were from HIS Statistical Integration. The long length-of-stay in hospital was defined as more than 30 days. Unconditional logistic regression was used for influence factors of long length-of-stay in the department of rehabilitation medicine. Results Multivariate logistic regression analysis showed that influence factors included ： transfer （ OR = 31.65 ）, nosocomial infections （OR= 3.19） , neurological disease and its sequelae （OR= 2.84）, complications （OR= 1.49）, waiting days before surgery （OR = 1.08）. Conclusion Length of stay of department of rehabilitation medicine is influenced by multiple factors, and prevention measures would be taken to control long length-of-stay.
