TPS术中指导在^(125)I放射性粒子植入治疗中的价值

VALUE OF TPS GUIDANCE DURING ^(125)I SEED IMPLANTATION THERAPY

目的探讨治疗计划系统(TPS)术中指导对CT引导下125I放射性粒子治疗中晚期肺癌的临床价值。方法 40例中晚期肺癌病人,均行CT引导下经皮穿刺植入125I粒子治疗,其中20例未进行TPS术中指导(无术中指导组),20例进行TPS术中指导(术中指导组)。比较两组90%肿瘤体积接受剂量(D90)、平均中心剂量(MCD)和匹配周缘接受剂量(MPD)。结果术中指导组术后MCD、MPD、D90与术前计划比较差异均无显著性(P>0.05);无术中指导组术后各指标与术前计划比较差异均有显著性(t=3.54~17.91,P<0.05)。结论 TPS术中指导使靶区剂量分布均匀且达到了术前处方剂量。

Objective To assess the value of TPS monitor during 125I seed implantation therapy under CT guidance for midlate stage lung cancer. Methods Forty patients with midlate stage lung cancer were treated with CT guided percutaneous 125I seed implantation. Of whom, 20 were monitored by TPS, while the other 20 were not. The detection indexes, including Dg0 , MCD and MPD, were compared between the two groups. Results For TPS-monitor group, the postoperative MCD, MPD and Dg0 were not significantly different as compared with that before operation （P〉0.05） ; For non TPS-monitor group, the differences of the above indexes between before and after surgery were significant 0-3.54 17.91,P〈0.05）. Conclusion Intraoperative TPS monitor makes even dose distribution in target area and reaches preoperative prescription dose.
