胶原蛋白人工硬膜在神经微血管减压术中的疗效研究

摘要 目的探讨胶原蛋白人工硬膜在神经微血管减压术中的疗效。方法将因三叉神经痛接受神经微血管减压术(MVD)治疗的患者100例,随机分为观察组和对照组,每组各50例。MVD术中,对照组采用自体筋膜修补硬脑膜缺损,观察组采用胶原蛋白人工硬膜修补。观察两组患者术后头晕、恶心、呕吐以及有无术后感染、脑脊液漏、皮下积液、蛛网膜粘连等并发症的发生,并进行比较分析。结果术后两组患者头晕、恶心、呕吐等一般并发症,以及术后感染发生例数比较差异无统计学意义;但观察组术后脑脊液漏、皮下积液、蛛网膜粘连等并发症的发生例数显著低于对照组(P〈0.05)。结论胶原蛋白人工硬膜可作为替代材料,用于三叉神经微血管减压术后硬脑膜缺损的修复,其疗效显著优于自体筋膜组织。 Objective To investigate the efficacy of collagen protein as an artificial dura graft on nerve microvascular decompression. Methods One hundred patients with trigeminal neuralgia under-went microvascular decompression (MVD) were randomly divided into observation group and control group, with 50 cases in each group. In the surgery, autologous fascia was used to repair dural defects in control group and patients in observation group were given collagen protein as an artificial dural. The gen- eral complications of dizziness, nausea and vomiting were observed. Other indicators including postopera- tive infection, cerebrospinal fluid leakage, subcutaneous fluid and arachnoid adhesions also were ob-served and compared. Results There was no significant difference in postoperative infection, dizziness, nausea, vomiting between the two groups after surgery. However, the numbers of cases who had cerebro-spinal fluid leakage, subcutaneous fluid, and arachnoid adhesions were significantly lower in observation group than those in control group (P 〈 O. 05 ). Conclusions Collagen protein artificial graft can be used as alternative materials to repair the dural defects after microvascular decompression for tri- geminal neuralgia, and its efficacy is significantly superior to autologous fascia tissue.
