心理暗示对前列腺炎样症状转归的影响

Effect of Psychological Suggestion on the Trajectory of Prostatitis-Like Symptoms

目的:了解心理暗示对前列腺炎样症状的转归有无影响。方法:将符合纳入标准的有前列腺炎样症状的患者分为受到心理暗示组和未受心理暗示组,2组患者在初次就诊时均被告知症状可能自行缓解,复诊时症状未完全缓解者给予适当的对症治疗。随访3个月,分别统计2组患者慢性前列腺炎(chronic prostatitis,CP)的发生率及年龄构成。结果:纳入患者169例,失访15例。其中受到心理暗示组49例,未受心理暗示组105例,2组患者最终诊断为CP者分别为43例和41例。受到心理暗示组CP发生率(87.8%)显著高于未受心理暗示组(39.0%),差异有统计学意义(χ2=31.97,P<0.01);受到心理暗示组发展为CP的危险性高于未受心理暗示组,相对危险度为2.2。2组患者的年龄构成中,受到心理暗示组16～25岁年龄段的患者所占比例(46.9%)显著高于未受心理暗示组的比例(23.8%),差异有统计学意义(χ2=9.39,P<0.01)。结论:心理暗示对前列腺炎样症状的转归存在负面影响,其中对青少年患者的影响更为明显。

Objective：To evaluate the effect of psychological suggestion on the trajectory of prostatitis-like symptoms.Methods：Patients met the inclusion criteria were divided into two groups：the group suffered from psychological suggestion and the group not suffered from psychological suggestion.All patients in two groups were told at their first visit that their symptoms might be relieved spontaneously.Those whose symptoms were not relieved completely were given an appropriate symptomatic treatment at their follow up visit.All patients were followed up for 3 months,and the incidence and age structure of patients with chronic prostatitis（CP） in two groups were calculated respectively.Results： A total of 169 patients were included,and 15 patients were lost to follow up.The incidence of CP in the group suffered from psychological suggestion（87.8 %,43/49）was significantly higher than that in the group not suffered from psychological suggestion（39.0 %,41/105）（χ2=31.97,P0.01）,and the group suffered from psychological suggestion had a higher risk of progressing to CP than the group not suffered from psychological suggestion with a relative risk of 2.2.There was a higher proportion of patients aged 16 to 25 years in the group suffered from psychological suggestion（46.9 %） than that in the group not suffered from psychological suggestion（23.8 %）（χ2=9.39,P0.01）.Conclusions： There may be a negative effect of psychological suggestion on the trajectory of prostatitis-like symptoms,especially for the adolescents with prostatitis-like symptoms.
