摘要
目的:探讨螺旋CT血管成像(spiral CT angiography,SCTA)的曲面重建(curved planar reconstruction,CPR)在腹主动脉瘤(abdominal aortic aneurysm,AAA)患者腔内修复术(endovascular aneurysm repair,EVAR)前评估中的价值。方法:回顾分析2012年11月—2013年3月35例AAA患者的SCTA资料及数字减影血管造影(digital subtraction angiography,DSA)资料,这些患者均在术前采用CPR、多平面重建(multi-planar reformation,MPR),术中采用DSA检测瘤颈长度、瘤体长度、瘤颈内径等AAA相关指标,比较各方法间结果的差异。结果:CPR法检测的瘤颈长度及瘤体长度之和与DSA法检测的结果之间差异无统计学意义(P=1.000),而MPR法检测的瘤颈长度及瘤体长度之和与DSA、CPR法相比,差异均有统计学意义(P均<0.05)。结论:CPR可以准确测量AAA患者的各解剖学参数,为临床上AAA患者进行EVAR提供帮助。
Objective:To explore the application value of spiral CT angiography(SCTA)curved planar reconstruction(CPR) in preoperative assessment of endovascular aneurysm repair(EVAR)in patients with abdominal aortic aneurysm(AAA).Methods:SCTA and digital subtraction angiography(DSA)datum of 35 patients with AAA from November 2012 to March 2013 were retrospectively analyzed.The parameters of AAA,including lengths of the aneurysm neck and body,as well as the diameter of aneurysm neck were measured with preoperative CPR,multi-planar reformation(MPR)and intraoperative DSA.The differences of the three methods were compared.Results:No significant difference in the sum of lengths of the aneurysm neck and body was found between CPR and DSA results(P=1.000).However,the sum of lengths of the aneurysm neck and body measured with MPR was significantly shorter than that measured with CPR and DSA(P〈0.05).Conclusions:CPR can accurately measure the anatomical parameters of AAA,which is helpful to EVAR in clinical practice.
