急性颅脑创伤对下丘脑-垂体-肾上腺皮质轴激素的影响 被引量:2

摘要 目的研究急性颅脑创伤对患者下丘脑-垂体-肾上腺皮质轴激素的影响,了解颅脑创伤后机体的内分泌反应以及相关激素的变化。方法采用化学发光分析法对58例颅脑创伤患者血清促肾上腺皮质激素(ACTH)、总皮质醇(COR)水平进行测定,应用SPSS16.0统计软件进行分析研究。结果轻中型颅脑创伤患者ACTH、COR多处于正常水平,重型颅脑创伤患者ACTH、COR显著升高。结论颅脑创伤后,多数轻中型颅脑创伤患者下丘脑-垂体-肾上腺皮质轴功能多保持完整,但重型颅脑创伤患者血清ACTH、COR显著升高,约90%的患者可出现不同类型皮质醇分泌异常。 Objective To research the alteration of hypothalamus-pituitary-adrenal cortex axis(HPA axis) functions after traumatic brain injury and observe the endocrine responses represented by related hormones.Methods Chemiluminescent immunometric assay was applied to measure serum ACTH and total cortisol(COR),SPSS16.0 statistical package was used for data analysis.Results ACTH and COR were within the normal range in mild and moderate trauma group,while in severe injury group,most patients presented elevated ACTH and COR.Conclusion After traumatic brain injury,most patients with mild and moderate injury were with integrated HPA axis function,while 90% severe injuried patients showed an abnormal HPA axis function,with raised ACTH and COR.
