中枢神经细胞瘤的MRI表现与病理的对比研究

摘要 目的分析并探讨中枢神经细胞瘤(CNC)的磁共振成像(MRI)与病理表现特点。方法回顾性分析广西柳州钢铁(集团)公司医院于2008年12月至2011年12月收治的13例CNC患者的MRI资料及病理资料,所有患者均进行血管内皮生长因子与微血管密度(MVD)测定,分析比较CNC患者与胶质瘤患者的检测结果。结果 CNC组MRI平扫13例T1WI均呈对等或较低信号;12例T2WI呈等信号或是较高信号。病理上肿瘤细胞呈弥漫性分布,瘤细胞大小一致性程度较高,胞质少,细胞核小,染色质颗粒较粗,可见瘤细胞围绕血管呈现出袖套样浸润,均未出现明显的出血,部分出现钙化现象。胶质瘤组患者MVD值为45.1±15.5,CNC的MVD值为20.9±10.5(P<0.01)。结论CNC的病理学基础决定其MRI的表现具有一定的特征性,采用影像学检查与临床资料结合,可提高临床诊断准确率。 Objective To analyze and discuss the magnetic resonance imaging(MRI) and pathological characteristics of central nervous cell tumor(CNC). Methods The MRI data and pathologic data of 13 cases CNC patients admitted by Liuzhou Iron and Steel(Group) Company Hospital from December 2008 to Decem-ber 2011, were retrospectively analyzed, all patients were tested for vascular endothelial growth factor (VEGF) and microvascular density( MVD), results of the CNC patients and gliomas patients were analyzed and compared. Results The 13 cases MRI fiat scanning showed peer or low T1WI signal. 12 cases showed peer or high T2WI signal. Pathologically tumor ceils showed the diffuse distribution, tumor cell size consisten-cy degree high, less cytoplasm, nuclear small, chromatin particle thicker, and the visible tumor cells around vascular presented a sleeve sample infiltration, all without obvious bleeding, some with the phenomenon of calcification. Malignant glioma patients MVD value was 45.1 ± 15.5, CNC MVD value was 20. 9±10.5 ( P 〈 0.01 ). Conclusion CNC pathology basis determines that its MRI performance has a certain characteris-tics ,the combination of imaging examination and clinical information can improve clinical diagnosis accuracy.
