精细化被膜解剖技术在双侧甲状腺手术中的应用 被引量:3

Application of meticulous capsular dissection in bilateral thyroid operation

摘要 目的探讨精细化被膜解剖技术(MCD)在双侧甲状腺手术中的应用效果。方法双侧甲状腺良性疾病手术患者295例分为两组:A组189例,采用MCD;B组106例,采用非MCD;比较两组术后并发症发生情况。结果两组均顺利完成手术。A组术中失血量少于B组[(39.5±10.9)ml vs.(55.0±18.2)ml](P<0.05)。A组一过性喉返神经麻痹发生率低于B组(2.6%vs.8.5%)(P<0.05)。A组一过性甲状旁腺功能减退的发生率亦低于B组(6.9%vs.17.9%)(P<0.05)。结论采用MCD能够减少双侧甲状腺手术的并发症。 Objective To investigate the outcomes of application of meticulous capsular dissection(MCD) in bilateral thyroid operation. Methods A total of 295 patients undergoing the surgery for bilateral benign thyroid disease was divided into two groups of A(189 cases, application of MCD) and B( 106 cases, application of non-MCD). The postoperative complications were compared. Results The surgeries were all successful in both groups. The intraoperative blood loss in group A was less than that in group B[(39.5±10. 9) ml vs. (55.0±18.2) ml](P〈0. 05). The incidence rate of transient recurrent laryngeal nerve palsy after operation was lower in group A than that in group B (2.6% vs. 8.5%) (P〈0. 05). So did the incidence rate of transient hypoparathyroidism ( 6.9 vs. 17.9%)(P〈0. 05). Conclusion The bilateral thyroid operation with MCD technique may decrease the complications of intraoperative blood loss and postoperative incidence rates of transient recurrent laryngeal nerve palsy and transient hypoparathyroidism.
