清热消肿方对上肢闭合性骨折瘀热证影响30例临床研究

Clinical Study on the Effect of Prescription for Heat- clearing and Swell- reducing on Upper Limb Closed Fractures Patients with Stasis Heat Syndrome

目的观察自拟清热消肿方对上肢闭合性骨折早期瘀热证的临床疗效。方法自2011年6月-2012年7月在佛山市中医院住院治疗上肢闭合性骨折（肱骨外科颈、肱骨干、肱骨髁上、髁间、尺桡骨骨折）患者中，选取符合纳入标准而无排除标准的病例60例。男37例、女23例，年龄21～50岁，平均（37．8±2．5）岁；其中肱骨外科颈12例、肱骨干9例、肱骨髁上9例、髁间11例、尺桡骨骨折19例；伤后至就诊时间为（3．8±1．7）h。将上述60例患者随机分为2组各30例。2组从入院第1天开始在常规治疗基础上分别每天给予清热消肿方、桃红四物汤各l剂。并于入院第1d（给药前）、第3d、第5d、第7d采集瘀热证临床症状积分，单独采集疼痛、肿胀、便秘积分。结果2组第1天瘀热证临床症状、疼痛、肿胀、便秘积分水平比较无显著性差异（P〉0．05），具有可比性；而在第3d、第5d、第7d的比较差异均有统计学意义（P〈0．05）。结论清热消肿方能明显改善上肢闭合性骨折瘀热证患者的临床症状，并在止痛、消肿、通便等方面有明显疗效。

Objective: To observe the clinical effect of self- made heat- clearing and swell- reducing prescription for upper limb closed fractures patients with early stasis heat syndrome. Methods: From June,2011 to July,2012,60 upper limb closed fractures inpatients( surgical neck of humerus,humeral,condyle,radius and ulna fractures) were selected. There were 37 males and 23 females,aged from 21 to 50,mean 37. 8 ± 2. 5 years. Among them,12patients with surgical neck of humerus,9 with humeral shaft fracture,9 with supracondylar fracture,11 with intercondylar fracture and 19 with radius and ulna fracture,came to have treatment after 3. 8 ± 1. 7 hours of their injury. The above 60 patients were randomly divided into 2 groups,30 patients per group. In the first day when they were hospitalized,the patients were daily treated with the self- made heat- clearing and swell- reducing prescription besides conventional treatment. Their clinical symptom scores and their pain,swell and constipation scores were separately collected in the 1st day( pre- treatment),in the 3rd,5th and 7th day. Results: The comparison of the scores of their clinical symptom,pain,swell and constipation showed no significant difference( P> 0. 05) in the 1st days and was comparable,but the comparison of the scores in the 3rd,5th and 7th day showed statistically significant difference( P <0. 05). Conclusion: The prescription for heat- clearing and swell- reducing can significantly improve the clinical symptom of the upper limb closed fractures patients and has a significant effect on pain- relieving,swell- reducing and bowel- relaxing.
