老年2型糖尿病患者三餐加睡前胰岛素强化治疗使用地特胰岛素优于中性鱼精蛋白锌胰岛素

Meal-time insulin aspart plus bedtime determir was superior to plus bedtime NPH in the elderly T2DM patients

目的观察预混胰岛素血糖控制不佳的老年T2DM患者使用地特胰岛素联合门冬胰岛素的疗效。方法选取预混胰岛素血糖控制不佳的老年T2DM患者60例,分为地特胰岛素治疗(A)组及中性鱼精蛋白锌胰岛素(NPH)治疗(B)组各30例,观察两组治疗前后血糖变化、每日胰岛素用量、血糖达标时间和低血糖发生率。结果两组治疗后血糖均下降。与B组相比,A组血糖变化更平稳,血糖达标时间更短,每日胰岛素用量更少,低血糖发生率更低,差异有统计学意义。结论老年T2DM患者需(?)餐加睡前胰岛素强化治疗时,三餐前门冬胰岛素联合睡前地特胰岛素优于睡前NPH。

Objective To observe the clinical effect of insulin detemir in combination with insulin aspart on the elderly T2DM patients with poorly controlled blood glucose. Methods Totally 60 elderly T2DM patients with a poor response to premixed insulin were randomized into insulin detemir group （group A） and insulin neutral protamine hagedorn （NPH） group （group B） with 30 in each. Group A was treated with insulin detemir ＋ insulin aspart, and group B with insulin NPH ＋ insulin aspart. The changes in the level of blood glucose, insulin dosage, the time to target blood glucose, and the incidence of hypoglycemia of the two groups were observed and compared. Results The level of blood glucose of both groups after treatment was significantly lowered. As comparing with Group B, the decrease of blood glucose was faster, the daily insulin dosage was lower, the time to the target blood glucose was shorter, and the incidence of hypoglycemia was lower in group A, and the difference is statistically significant. Conclusion The effect of insulin detemir in combination with insulin aspart is superior to insulin NPH plus insulin aspart on elderly T2DM patients.
