平衡法门控心血池显像对左右心室功能的评估

摘要 目的评价平衡法门控心血池显像(GBPI)在左右心室收缩功能中的临床价值。方法对18例心脏病患者在2w内进行GBPI和实时三维超声心动图(RT-3DE)检查,以RT-3DE为参照标准,分别对两种方法测定的左室射血分数(LVEF)和右室射血分数(RVEF)进行分析和Bland-Altam一致性检验。结果 GBPI与RT-3DE测量的LVEF、RVEF比较,差异无统计学意义;Bland-Altam分析表明GBPI与RT-3DE在测定LVEF和RVEF方面具有良好的一致性,但二者测定RVEF的一致性略低于测定LVEF。结论 GBPI一次成像可准确反映左右心室收缩功能,是一种安全简便的评价左右心室功能的无创方法,具有重要的临床价值。 Objective To explore the clinical value of gated blood pool imaging (GBPI) on the left and right ventricular systolic function. Methods Within 2 weeks, 18 patients with heart disease were tested by GBPI and real time three-dimensional echocardiograpby (RT-3DE). With RT-3DE as the reference standard, the left ventricular ejection fraction (LVEF) and right ventricular ejection fraction (RVEF) were respectively analyzed and subjected to Bland-Altam consistency test for these two methods. Results Compared with LVEF and RVEF measured by GBPI and RT-3DE, there was no significant difference. Bland-Altam analysis demonstrated that both GBPI and RT-3DE had better consistency in monitoring LVEF and REVF, but the consistency of RVEF measured by these two methods was slightly lower than that of LVEF. Conclusion An instant imaging of GBPI can accurately reflect the left and right ventricular systolic function, which is a safe, convenient and non-invasive method in assessing the left and right ventricular function and thus has important clinical values.
