复方亚甲蓝不同注射方式用于痔术后止疼效果的研究 被引量:19

摘要 目的观察长效止痛剂亚甲蓝用于痔疮术后不同注射方式的止痛效果。方法采用随机平行对照的研究方法,根据手术指征选择60例混合痔患者,将其分为观察组和对照组,各30例。两组术式均为外剥内扎术,观察组于术后在肛周行菱形浸润注射;对照组于术后在切口边缘封闭注射。于术后各时间点观察术后创面疼痛、水肿及愈合时间等指标。结果术后2 h观察组疼痛程度显著低于对照组(P<0.01);术后6 h观察组止痛效果优于对照组(P<0.05);术后12、24、72 h两组显效率比较,差异无统计学意义(P>0.05);两组不良反应发生率(暂时性肛门失禁、局部水肿、尿潴留、排便意识障碍、创面愈合时间等)比较差异无统计学意义(P>0.05)。结论观察组采用的注射方式所产生的止痛效果,尤其术后早期止痛效果明显优于对照组,是长效止痛制剂亚甲蓝的理想注射方式。 Objective To observe the analgesic effects of different injection methods of compound methylene blue on hemorrhoid postoperative pain.Methods 60 patients with mixed hemorrhoid were selected based on surgical indications,they were divided into observation group (n =30)and control group (n =30)by a random and parallel-controlled method.Milligan-Morgan hemorrhoidectomy was used in 2 groups.After operation,diamond infiltration injection in perianal was used in observation group,and closed injection at the incision edge was used in control group.Indications(such as wound pain,edema,and healing time etc.)were observed at different time points after operation.Results The severity of pain in observation group was significantly lower than that of control group at 2 h(P < 0.01)and 6 h (P <0.05)after surgery.There was no significant difference in pain severity at 12 h,24 h and 72 h after surgery between the two groups(P > 0.05).No significant difference was observed in the incidence of adverse reactions(temporary anal incontinence,local edema,urinary retention,bowel disorders of consciousness,wound healing time etc.) between the two groups (P > 0.05).Conclusion The analgesic effect of the injection method used in observation group (particularly its early analgesic effect) is significantly better than that of control group.It is an ideal injection method of long-acting analgesic methylene blue.
