侧脑室穿刺并腰大池置管持续引流净化脑脊液治疗脑室铸型出血的研究

The Study of Lateral Ventricle Puncture and Purification of Cerebrospinal Fluid by Lumbar Cistern Continuous Drainage in the Treatment of Total Intraventricular Hemorrhage

目的研究侧脑室穿刺并腰大池置管持续引流净化脑脊液治疗脑室铸型出血的疗效。方法对60例脑室铸型出血患者进行侧脑室穿刺并腰大池置管持续引流净化脑脊液治疗(联合治疗组)及单纯脑室穿刺引流治疗(单纯脑室引流组)的随机对照研究,以比较两种疗法治疗患者的死亡率、头痛消失时间、脑膜刺激征消失时间、意识障碍消失时间及交通性脑积水的发生率。结果联合治疗组患者死亡率、头痛消失时间、脑膜刺激征消失时间、意识障碍消失时间及交通性脑积水的发生率均与单纯脑室引流组存在显著性差异。结论侧脑室穿刺并腰大池置管持续引流净化脑脊液治疗脑室铸型出血简单易行,疗效确切,安全可靠,值得在临床推广。

Objective To study the efficacy of lateral ventricle puncture and purification of cerebrospinal fluid through lumbar cistern continuous drainage in the treatment of total intraventricular hemorrhage.Methods 60 cases of patients with total intraventricular hemorrhage admitted in our hospital from March,2006 to August,2012 were randomly divided into the combined treatment group and the simple ventricular drainage treatment group with 30 cases in each.The combined treatment group was given lateral ventricle puncture and purification of cerebrospinal fluid through lumbar cistern continuous drainage.The simple ventricular drainage treatment group was given ventriculopuncture.The mortality,disappearance time of headache,meningeal irritation sign and disturbance of consciousness,and the incidence of communicating hydrocephalus of the two groups were compared.Results There were significant differences between the two groups in terms of mortality,disappearance time of headache,meningeal irritation sign and disturbance of consciousness,and the incidence of communicating hydrocephalus.Conclusion Lateral ventricle puncture and purification of cerebrospinal fluid through lumbar cistern continuous drainage in the treatment of total intraventricular hemorrhage is easy to operate,effective,safe and reliable,which is worth of clinical promotion.
