肠系膜上动脉供血型肝癌的DSA探讨

Discussion on Digital Substraction Angiography of Hepatocelluar Carcinoma Supplied by Superior Mesenteric Artery

目的探讨肠系膜上动脉供血型肝癌的数字减影血管造影(DSA)表现及造影技术。方法回顾分析55例肠系膜上动脉供血型肝癌的DSA表现,统计血管解剖变异的发生率,明确供血动脉的起始部位、走行及供血部位。结果 420例肝癌中肠系膜上动脉供血型55例(13.1%)。其中替代肝右动脉22例(40%),副肝右动脉20例(36.4%),肝总动脉9例(16.4%),腹腔干起源于肠系膜上动脉4例(7.2%)。结论对于肝肿瘤(特别是肝右叶肿瘤)在行DSA和介入治疗时应常规作肠系膜上动脉造影。

Objective To investigate the digital substraction angiography manifestation and technique of hepatocelluar carcinoma supplied by superior mesenteric artery.Methods We analyzed digital substraction angiography manifestation of 30 patients suffering hepatocelluar carcinoma supplied by superior mesenteric artery retrospectively,calculated the incidence of anotomic variation,and confirmed the start position,distribution,supplying tissue in superior mesenteric artery.Results There were 55 patients whose tumors were supplied by superior mesenteric artery in total 420 heptocelluar carcinoma patients（13.1%）.Superior mesenteric artery took the place of right hepatic artery in 22 patients（40%） and took the place of accessory right hepatic artery in 20 patients（36.4%）,took the place of common heaptic artery in 9 patients（16.4%）;4 patients（7.2%） underwent a stem vascular of coeliac trunk artery and superior mesenteric artery.Conclusions We suggest taking the digital substraction angiography manifestation of hepatocelluar carcinoma especially in right lobe as routine examination.
