大鼠结肠黏膜电切术后自然愈合过程中天然免疫的变化

摘要 目的探讨部分天然免疫在大鼠结肠黏膜切除术后溃疡局部的变化。方法选取36只成年健康SD大鼠随机分成3组:正常对照组、假手术组(分为4天、7天、12天3个时间段小组)和电切组(分为4天、7天、12天3个时间段小组)。除正常对照组外,其余各组动物均接受剖腹手术,电切组行结肠黏膜电切术;假手术组行结肠黏膜全层切开而不行结肠黏膜电切;正常组只适应性喂养1周后处死。用免疫组化法(SABC)对结肠相应部位组织中CD177、CD163和CD159a行免疫组化染色,用图像分析法测定各指标阳性表达部位的积分光密度值(IOD)。结果电切组、假手术组及正常组大鼠结肠组织中均有CD177、CD163和CD159a的表达。阳性染色主要显色于黏膜上皮层及黏膜固有层中,少数也显色于黏膜下层中。电切组中的CD177和CD163在各时间段肠道溃疡局部组织的IOD值较假手术组和正常组均增加(P<0.05),电切组内各时间段之间没有差异(P>0.05)。各组之间及各时间段的CD159a的IOD值均没有差异(P>0.05)。结论大鼠肠黏膜电切术后至少12天内,局部存在急性炎性反应。参与天然免疫的中性粒细胞、单核/巨噬细胞浸润明显增多,其中单核/巨噬细胞增多的最终效应复杂、不清楚,而NK细胞的数量变化不明显。 Objective To investigate the changes of innate immunity around the wound in rats with colonic mucosal electroresection.Methods 36 SD rats were randomly divided into 3 groups: normal control group,sham group(divided into 4thday,7thday,12thday subgroup) and electrocision group(divided into ulcer group 4thday,ulcer group 7thday,ulcer group 12thday subgroup).All rats in each group except normal control underwent laparotomy and colonic mucosa resection.Each rat in electrocision group received mucosal electrocision at colon one centimeter near the appendices.Sham group received the same operation as electrocision group except colon mucosal enectrocision.Every 4 rats were executed at the 4thday,7thday,12thday in electrocision group and sham group.The normal control group was executed after one week.Tissue simples at the mucosal electrocision site of colon in electrocision group,and those at the similar site of colon in sham group and normal control group,were prepared for paraffin section which was used to detected CD177,CD163 and CD159a by immunohistochemistry and image analysis.Results The positive staining was mainly located in the colon mucosa epithelium layer and lamina propria;a light positive staining was found in the submucosa in all groups.IOD of CD177 and CD163 in electrocision group were increased compared with sham group and normal control group(P 0.05),and no difference among each time point of electrocision group(P 0.05).There were no differences of IOD of CD159a among the groups and each time subgroups(P 0.05).Conclusions Acute inflammatory reaction in the site of mucosal electrocision lastes for at least 12 days.The cell participate in acute inflammatory reaction include monocytes / macrophages and neutrophil increase markedly.The effect of macrophages is unclear.There is no obvious change of NK cells.
