摘要
目的探讨联合检测α-1,6岩藻糖基转移酶(FUT8)、岩藻糖基化的甲胎蛋白(AFP-L3)和α-L-岩藻糖苷酶(AFU)在原发性肝癌(PLC)诊断中的应用价值。方法选择92例PLC患者(PLC组)和42例非原发性肝癌患者(NPLC组),采用ELISA法检测血清FUT8和AFP-L3,采用电化学发光法检测血清AFU。结果 PLC组血清FUT8阳性62例(67%)、AFP-L3阳性69例(75%)、AFU阳性65例(71%),NPLC组分别为1例(2%)、3例(7%)、5例(12%);两组比较,P均<0.05。FUT8诊断PLC的敏感性为67.39%、特异性为76.87%、准确性为91.04%,AFP-L3分别为75.00%、92.86%、80.60%,AFU分别为70.65%、88.10%、76.12%,三者联合检测分别为91.30%、90.48%、97.61%,三者联合检测诊断PLC的敏感性、准确性与单项检测比较,P均<0.05。结论血清FUT8、AFP-L3和AFU联合检测有助于PLC的诊断。
Objective To investigate the application value of combined detections of or-1,6-fucosyltransferase (FUT8), alpha-fetoprotein variant (AFP-L3) and α-L-fucosidase (AFU) in the diagnosis of primary liver cancer (PLC). Methods The enzyme linked immunosorbent assay (ELISA) was applied to detect the serum FUT8 and AFP-L3 in 92 cases of patients with PLC patients and 42 cases of patients with non-primary liver cancer ( NPLC), and chemiluminescence immunoassay was used to detect the serum AFU. Results In PLC group, there were 62 (67%) positive cases for FUT8, 69 (75%) for AFP- L3 and 65 (71%) for AFU, however, it was 1(2% ), 3(7% ) and 5(12% ) in NPLC group. Through statistical analysis, serum FUTS, AFP-L3 and AFU had significant differences between PLC and NPLC group ( all P 〈 0. 05). In diagnosis of PLC , the sensitivity of FUT8 was 67.39%, the specificity was 76.87% and the accuracy was 91.04%, they were 75.00%, 92.86% and 80.60% for AFP-L3, and they were 70.65%, 88.10% and 76.12% for AFU, but in the combined detections, they were 91.30%, 90.48% and 97.61%. Through statistical analysis, the combined detections were better than any individual detection in the diagnosis of PLC for sensitivity, specificity and accuracy (all P 〈 0. 05). Conclusion The combined detections of serum FUT8, AFP-L3 and AFU are useful for the diagnosis of PLC.
