血浆N末端B型脑利钠肽在心力衰竭诊断中的价值 被引量:8

摘要 目的 探讨血浆N末端B型脑利钠肽(NT-proBNP)在心力衰竭患者中的诊断价值.方法 将110例心力衰竭患者分为代偿组(NYHAⅠ ~Ⅱ级,n=41)和失代偿组(NYHAⅢ~Ⅳ级,n=69),再根据射血分数大小分为左心室射血分数(LVEF)≤40%组(n=31)和LVEF> 40%组(n=79).采用罗氏方法测定血浆NT-proBNP.结果 患者血浆NT-proBNP水平与美国NYHA心功能不全的分级标准密切相关(P<0.01),心功能不全越严重血浆NT-proBNP值越高;与LVEF值呈负相关,LVEF值越低,NT-proBNP越高(P<0.01).失代偿组血浆NT-proBNP水平高于代偿组(P<0.01);LVEF≤40%组血浆NT-proBNP水平高于LVEF> 40%组(P<0.05).受试者工作特征曲线下面积(AUC)心力失代偿组为0.831,LVEF≤40%组为0.931.在失代偿组ROC曲线中,当NT-proBNP取切点值为1195 ng/mL,诊断心力衰竭的敏感性、特异性和准确性分别为73.9%、79.9%和78.3%,阳性预测值和阴性预测值分别为65.9%和83.3%.在LVEF≤40%组ROC曲线中,NT-proBNP取切点值2 779 ng/mL,诊断的敏感性、特异性和准确性分别为90.9%、87.9%和82.8%,阳性预测值和阴性预测值分别为96.1%和94.3%.结论 血浆NT-proBNP值可作为判断心力衰竭程度的指标,尤其是对失代偿心力衰竭及LVEF严重受损有重要的诊断价值.
