河南地区121例丙型肝炎病毒基因型分析

Hepatitis C virus genotyping analysis among 121 patients in Henan province

目的了解河南人群中丙型肝炎病毒(HCV)的基因型分布及感染危险因素,为该省HCV临床治疗和估计HCV造成疾病负担提供参考信息。方法调查河南省62个村/居委会的常住人群(居住时间在6个月以上者)得到HCVRNA阳性标本121份,通过RT-PCR扩增NS5B基因,进行核苷酸序列测定,运用DNASTAR、BioEdit和Mega 5.1等软件作序列分析和分子进化分析。结果对河南地区121例基因型分析,1b占59.5%(72份)、6a占17.4%(21份)、3a占10.7%(13份)、2a占9.9%(12份)、3b占1.7%(2份),1a占0.8%(1份)。121例HCV RNA阳性标本中,有手术史者44例占36.4%,有修脚行为者36例占29.8%,不安全注射行为者23例占19.0%,有偿供血史者16例占13.2%。结论河南地区HCV感染方式多样化,基因亚型以1b和6a型为主,3a,2a,3b和1a等多种型别分布的特点。

Objective To study the distribution of hepatitis C virus （HCV） genotypes and the risk factors in Henan and provide guidance for HCV clinical treatment and estimate the burden of hepatitis C infection. Methods One hundred and twenty one anti - HCV positive plasma samples were obtained from 62 villages or neighborhood commit- tees in Henan who living more than six months in this study. Firstly, NSSB gene were detected by RT -PCR and phylogenetic analysis in HCV RNA positive samples. Futher, the NS5B region was amplified for sequencing analy- sis, and software Chromas Pro, Bio Edit and MEGA 5.1 were used to construct the phylogenetic tree. Results One hundred and twenty one samples were genotyped （ based on NS5B gene） , and subtypes HCVlb,6a,3a,2a,3b, la were detected at frequencies of 59.5%, 17.4% , 10.7%, 9.9%, 1.7%, 0.8%, respectively. In those 121 sam- pies, the number of people who having the history of surgery, pedicure behavior, unsafe injection behavior and paid blood collection was 44, 36, 23 andl6 at percentage of 36.4%, 29.8%, 19.0% and 13.2%. Conclusion The HCV infection was diverse in Henan. The most common HCV genotypes in patients with HCV infection were 1 b and 6a, followed by 3a, 2a, 3b and la, indicating that the genotypes of hepatitis C in this area might be diverse.
