糖尿病患者血糖控制水平与血清CEA的相关性研究 被引量:4

Correlation study on serum CEA level and glucose control in patients with diabetes mellitus

摘要 目的探讨糖尿病患者血糖控制水平与血清CEA的相关性并确定其参考临界值。方法收集144例住院糖尿病患者(分别进行强化治疗,以强化治疗后DM患者为DM组)、149例健康对照者(对照组)和149例消化道恶性肿瘤患者(肿瘤组)。比较DM患者强化治疗前后空腹血糖(FPG),餐后2 h血糖(PPG),癌胚抗原(CEA)和C反应蛋白(CRP)水平,绘制ROC曲线,以确定鉴别糖尿病参考临界值。结果强化治疗前患者CEA显著高于强化治疗后;DM组CEA显著高于健康对照者,消化道恶性肿瘤患者CEA显著高于DM患者;ROC曲线下面积0.878(95%可信区间:0.771~0.919),Cut-off值为7.05 ng/ml。结论DM患者的CEA显著高于健康对照者;糖尿病患者CEA参考临界值为7.05 ng/ml。 Objective To investigate the correlation between blood glucose control and serum carcinoembryonic an- tigen (CEA) level in diabetics, and find cut - off value of CEA between DM and digestive system cancer. Methods One hundred and forty - four hospitalized diabetics ( intensive hypoglycemic treatment) were collected as diabetes mellitus (DM) group after intensive hypoglycemic treatment, while the other two groups were 149 cases of healthy controls and 149 cases of gastrointestinal cancer patients. Levels of serum fasting plasma glucose (FPG) , 2 h post- prandial glucose ( PPG), CEA and C - reactive protein (CRP) in diabetics were compared before and after hypogly- cemic therapy. The levels of serum CEA were compared among the three groups. Receiver operating characteristic curve ( ROC curve) was constructed to calculate the cut - off value. Results The serum CEA level of pre - treat- ment in diabetics was significantly higher than that after intensive treatment. The serum CEA level in diabetics was significantly higher than that in healthy controls, and the serum CEA level in gastrointestinal cancer patients was sig- nificantly higher than that in diabetics. Area under ROC curves was 0. 878 (95% CI: 0. 771 -0. 919) and the cut - off value of CEA between DM and digestive system cancer was 7.05 ng/ml. Conclusion The sera level of CEA in diabetics was significantly higher than that in healthy controls, the cut - off value of CEA in diabetics was 7.05
