摘要
目的:检测胞质多聚腺苷酸化成分结合蛋白4(CPEB4)和Bcl-xL在胶质瘤组织中的表达,并探讨二者及多种临床病理因素与患者预后的关系。方法:应用免疫组化染色检测165例不同级别胶质瘤(低级别69例,高级别96例)组织中CPEB4和Bcl-xL的表达情况,采用logistic回归分析探讨影响胶质瘤预后的危险因素。结果:高级别胶质瘤组织中CPEB4和Bcl-xL的阳性表达率均高于低级别胶质瘤组织(χ2=28.971和17.601,P<0.05)。单因素分析显示,患者术前KPS评分、肿瘤直径、肿瘤切除范围、术后放疗、术后化疗、病理级别、CPEB4和Bcl-xL的表达对3 a生存率有影响(P<0.05),构建的判别函数判别符合率达90.9%;多因素分析表明,术前KPS评分、肿瘤切除范围、术后化疗、病理级别、CPEB4和Bcl-xL的表达是影响胶质瘤患者3 a生存率的独立因素(P<0.05),判别函数Y=-0.644+1.386X3+1.052X5+1.248X6-2.679X10-1.042X11-2.659X12,判别符合率达88.3%。结论:CPEB4和Bcl-xL阳性表达与胶质瘤恶性程度有关;术前KPS评分、肿瘤切除范围、术后化疗、病理级别、CPEB4和Bcl-xL的表达可作为考察胶质瘤患者预后情况的重要指标。
Aim:To detect the expressions of cytoplasmic polyadenylation element-binding protein 4 ( CPEB4 ) and Bcl-xL in glioma tissue , and investigate the prognostic factors of patients with glioma .Methods:The expressions of CPEB 4 and Bcl-xL in 165 cases of different grade glioma (69 cases of low grade , and 96 cases of high grade ) tissue were detected by immunohistochemical staining .The univariate and multivariate logistic regression was used to analyze the relationship between the expressions of CPEB 4 and Bcl-xL and the clinical factors with 3-year survival rate of the patients .Results:The positive expression rates of CPEB 4 and Bcl-xL in high grade glioma were significantly higher than those in low grade glioma(χ2 =28.971 and 17.601,P〈0.05).The univariate logistic regression analysis screened 8 related factors(P〈0.05),among which, multivariate logistic regression analysis further screened 6 independent factors including preoperative KPS score, the resection extent , postoperative chemotherapy , malignant degree and the expressions of CPEB 4 and Bcl-xL (P〈0.05).Conclusion:The expressions of CPEB4 and Bcl-xL are related with the malignant degree of glioma .The pre-operative KPS score , the resection extent , postoperative chemotherapy , malignant degree and the expressions of CPEB 4 and Bcl-xL can be regarded as important prognostic indicators for patients with glioma .
