摘要 目的比较Visante-OCT、Sirius与A超测量中央角膜厚度(central corneal thickness,CCT)的差异性,评价三种仪器测量结果的一致性。方法分别用Visante-OCT、Sirius和A超测量43例屈光不正志愿者(均为右眼)的CCT,数据行单因素方差分析、简单线性相关及Bland-Altman分析。结果 Visante-OCT、Sirius和A超测量的CCT分别为(522.88±33.53)μm、(538.35±35.05)μm和(532.23±34.16)μm;三种仪器测量结果两两之间均有高度相关性;A超与Visante-OCT测量结果无统计学差异(P=0.208),测量差值为(9.35±7.39)μm,两者95%的一致性区间为(-3.4~22.1)μm;A超与Sirius测量CCT差异无统计学意义(P=0.409),测量差值为(-6.12±7.39)μm,两者95%的一致性区间为(-18.9~6.7)μm;Visante-OCT与Sirius测量CCT差异具有统计学意义(P=0.038),测量差值为(-15.47±7.39)μm,两者95%的一致性区间为(-30.2^-0.7)μm。结论 Visante-OCT与Sirius用于测量CCT,与A超的一致性较好,在临床使用中可替代A超,但需谨慎选择;Visante-OCT与Sirius测量CCT存在一定差异,两者不能替换使用。 Objective To evaluate the differences and agreement in central corneal thickness (CCT) measurements by Visante-OCT, Sirius and A-scan. Methods The CCTs of 43 healthy eyes were measured by Visante-OCT, Sirius and A-scan. The differences, correlations and agreements among three measurements were evaluated with one way analysis of variance (ANOVA), Pearson correlation coefficients and Bland-Altman plots respectively. Results The measured CCTs were (522. 88 ± 33.53)μm, (538. 35 ± 35.05)μm and (532. 23 ± 34. 16) μm respectively by Visante-OCT, Sirius and scan. There existed high pairwise correlations. The difference between A-scan and VisanteOCT was (9. 35 ± 7. 39)μm which was not significant (P = 0. 208), and the 95% limit of agreement was ( -3.4 - 22. 1 )μm. The difference between A-scan and Sirius was ( - 6. 12 ± 7. 39)μm which was not significant (P = 0. 409), and the 95% limit of agreement was ( - 18. 9 - 6. 7)μm. The difference between Visante-OCT and Sirius measurements was ( - 15.47 ± 7. 39)μm which was significant (P = 0. 038), and the 95 % limit of agreement was( - 30.2 - - 0. 7)μm. Conclusion Visante-OCT and Sirius have good coincidence with A-scan in measuring CCT and can replace A-scan in clinic, but cautious selection is needed. Visante-OCT and Siriu measurements have difference and can't replace each other.
